Urgent requests for appointments from your GP or specialists will be accommodated.
Revesby: Please call 02 9773 4308. The Entry is on River Rd. We are a 5 min drive from the Revesby (River Rd) Exit from the M5 motorway. Unlimited parking is available in Bebe Ave & Weston St. Limited parking is available in Hedlund St & Macarthur Ave. The nearest undercover parking is Coles at the Revesby Village Centre. Revesby station is 450m away (7 minute walk). Buses stop in front (923-Panania, Picnic Point, Revesby, Bankstown; 924- East Hills, Panania, Revesby, Bankstown).
Liverpool: Please call 02 9821 4654. Liverpool station is a 6 minute walk. Council parking is available via dewsbury lane (all day $13). Limited street parking is available on Moore St or College St. 3 hours free parking is available at Liverpool Westfield. Buses stop in front.
Gregory Hills: Please call 02 9054 9626. Parking is onsite, and accessed via Lasso Rd.
FEE POLICY
- Fees and applicable medicare rebates will be fully explained to you at the time of making a booking enquiry.
- Usually 30 mins is allocated to an initial consultation, giving enough time to properly assess your problems and set out a investigation and treatment plan;
- Usually 15 mins is allocated to a subsequent or follow-up attendance, and this costs half (50%) of the initial consultation fee. This is to follow-up the investigations and treatment plan, and to make adjustments as needed.
- In most instances, your medicare rebate will be returned into your account by medicare shortly after you have paid the fees (allow 30 mins). In some circumstances if the rebate cannot be immediately processed, you will be provided the necessary information to upload to Medicare or drop off at at a Medicare office.
- The cost of providing high quality specialist care is unfortunately not fully covered by medicare, and there will be some out of pocket expenses explained at the time of making the appointment.
- All accounts are payable in full on the day of consultation.
- Payment options include cash, eftpos or credit card (cheques are not accepted).
- Third Party fees relating to pathology, radiology or allied health (e.g. physiotherapy, occupational therapy, podiatry) are not determined by the practice but every care is made to provide options where these exist.
- A/Prof Thakkar does see worker’s compensation cases. Patients will need to arrange or provide a valid authorisation from the insurance company at the time of making the booking.
CANCELLATION POLICY
- Once an appointment has been made, exclusive time has been set aside for you.
- While we understand at times that urgent circumstances require the cancellation and rescheduling of appointments, we ask that you provide at least 48 hours cancellation notice by phone so that we can allocate your appointment to the next patient on the waiting list.
- Patients who cancel within 48 hours or simply do not attend may not be provided with further appointments.
WHAT TO BRING
1. A valid referral– this provides a summary of your key problems and enables you to claim a rebate from Medicare.
2. Relevant tests, investigations and reports
WHAT TO EXPECT
Consultations usually take 30-45 minutes for an initial consultation, and 15-20 minutes for a follow-up. Especially for your first consultation, please come 10 minutes early to allow enough time to complete registration paperwork. While we attempt to run to time, please be patient as consultations times can vary based on the complexity of the problems. The right time to sort out your problems will be provided.
As A/Prof Thakkar will usually examine you, try to wear loose, comfortable clothes to ensure that a thorough and accurate examination can be performed at your convenience.
